Korean Wrinkle Fillers: Blindness, Lumps and Aging Myths, Answered
Can filler cause blindness, stay in the skin for years or speed up aging? Dr. Ryu separates myth from real risk, and explains which areas need the most care.
Are Korean wrinkle fillers safe?
Hello, I'm Dr. Hyo Seop Ryu, a board-certified dermatologist (M.D., Ph.D.) and Chief Director at WELLS Clinic in Apgujeong, Seoul. I was previously an assistant professor at Seoul National University Bundang Hospital. Visitors often ask me whether Korean wrinkle fillers are safe after reading frightening stories online, and this article is based on my YouTube video from September 2026 above.
Filler is one of the earliest and most widely used cosmetic treatments, and serious complications are rare. The risk is real but it depends heavily on the area, the technique and the experience of the person injecting. Below I go through the three fears I hear most, then the areas that need the most care.
Can filler cause blindness?
Several cases of blindness after filler have been reported. The face has thousands of blood vessels, and around the nose and between the brows some of them lead toward the eye. If filler is accidentally injected into one of these vessels and blocks the supply to the eye, vision can be lost.
An injector who knows that anatomy well and uses a blunt cannula instead of a sharp needle can greatly reduce that concern. Because so many filler treatments are done every year, these complications do occur, but rarely, and most often in less experienced hands.
Know the warning signs. If your vision blurs or the skin turns unusually red after filler, contact the person who treated you straight away. Fast treatment keeps the damage to a minimum.
Does filler stay in the skin, or make you age faster?
Most hyaluronic acid fillers dissolve as designed: the softest are made to be absorbed in about six months, the firmest over one to two years. The body breaks them down with an enzyme called hyaluronidase while the water is gradually absorbed, helped by movement and circulation.
A plastic surgery journal reported cases where filler remained for years, which drew a lot of attention. That tends to happen in special situations, such as filler placed very deep in an area that moves little and has less circulation, like a deep nasolabial fold. For the areas most people treat, it isn't something to worry about.
Filler does not speed up aging. Skin that has been stretched by large, repeated amounts may take time to recover after the filler is dissolved, which can feel like sagging. Placed in small, appropriate amounts, filler can even stimulate the dermis to make more collagen. It depends on the method and the injector.
Dermal Fillers
- What it is
- Hyaluronic acid filler for the forehead, under-eye hollows, nasolabial folds, cheeks and chin, with volume planned for your face.
- How it works
- Dermatologists with over fifteen years of filler experience inject with a cannula where possible, which minimizes the vessel-damage risk of a standard needle. The product and amount are confirmed before treatment.
- Sessions
- Planned at consultation according to the areas treated.
- Recovery
- Confirmed at consultation. Side effects are possible, so every treatment starts with a dermatologist consultation.
- Price
- Confirmed at consultation
Why patients fly in for it
- Board-certified dermatologists inject in person, with a cannula where possible.
- The product and amount are confirmed with you before treatment.
- Nasolabial folds use a large-particle, long-lasting filler.

Message on WhatsAppWhich areas need the most care with Korean wrinkle fillers?
I don't think any area is strictly off-limits. But the centre of the face, between the brows, the nose and the nasolabial folds, is where academic papers report the most skin necrosis and retinal complications, because the vessels to the eye are well developed there.
- Inject into the correct skin layer, in the correct amount.
- Use a blunt cannula rather than a needle, and inject slowly.
- Choose an injector with real experience in these areas.
That is how we work on dermal fillers at WELLS. Our dermatologists use a cannula where possible, and we confirm the product and amount before treatment. For nasolabial folds we use a large-particle, long-lasting filler, and forehead volume is balanced so the face does not look larger. The price is confirmed at consultation.
Filler or a collagen stimulator?
Some doctors online say never to get filler. Filler still does things nothing else does quickly, and threads used instead carry their own risks, such as tissue reactions or a thread working its way out. The better question is what you want.
- Immediate volume and lift: filler is the most direct choice.
- Gradual, natural volume in hollow cheeks or temples: a collagen stimulator such as Sculptra Lifting, ₩1,320,000 per session (VAT excluded), builds collagen over months.
- A defined lift in a small area, such as the chin or forehead: filler remains the effective option.

Planning filler on a Seoul trip
Don't let fragments of internet advice scare you off. Every treatment has strengths and weaknesses; what matters is a proper consultation about the face you want, and an injector with the experience to prevent complications.
Send a photo, the areas you're concerned about and your travel dates on WhatsApp, and tell us about any filler or threads you've had before. You can read about my background on Dr. Hyo Seop Ryu's profile. Results vary, and side effects are possible, so every plan starts with a dermatologist consultation.
