Bad cheek fillers: what goes wrong, and how we plan cheeks
Bad cheek fillers usually come from the plan, not the product. A Seoul dermatologist on overfilling, hollow cheeks, dissolving, and lifting instead.
What do people actually mean by "bad cheek fillers"?
Almost everyone who searches for bad cheek fillers has one of two faces in mind: one that now looks wider and heavier than before, or one that looks puffy across the mid-face and still tired around the mouth. The complaint is rarely about the product itself. It is usually about where the volume went, and how much of it there was.
I have been injecting in Apgujeong since we opened WELLS in 2008, and the cheek is the area where the gap between *more volume* and *a younger face* is widest. Adding volume to a cheek that is sagging rather than hollow does not lift it — it loads it.
In practice, the results patients come in unhappy about fall into a short list.
- Too much product, so the face reads rounder and wider in photographs
- Product placed into the area that sags instead of the structure that supports it
- Visible lumps, an uneven surface, or one side sitting differently from the other
- Filler that looks fine at rest but bulges when you smile
- Rarely, a vascular event: any filler is injected into tissue with vessels running through it, and if product enters a vessel, blood flow can be disrupted, with colour change, severe pain and, in rare cases, tissue damage
Why filling the cheek that sags usually backfires
Ageing generally moves in two directions at once: hollowing and sagging. The useful rule is simple — fill what is genuinely hollow, and tidy up what sags. Problems start when a sagging lower cheek is treated as an empty one.
That is why, for mid-face heaviness, I often map the support structure instead of the soft spot. A newer version of this problem arrives with rapid weight loss. When weight drops quickly, the body reduces fat-rich areas first, and the face is strongly affected by its fat layer — so the natural direction of ageing gets a shortcut.
Front-cheek hollowing makes the nasolabial folds look deeper and flattens the mid-face, which is what reads as "tired". Side-cheek hollowing does something different: the cheek bone line stands out while the area beneath it empties, so the face can look pinched in the middle — the shape patients describe as peanut-like.
This is not simple weight loss. Volume loss, reduced elasticity and sagging are progressing together, which is exactly the situation where one syringe of filler in the wrong plane produces a result you will call bad.

One more thing that surprises people: below the cheekbone, the tissue is often strongly adherent. In those cases filler alone may not fully correct the hollow, and pushing more product in to force it is how lumps and heaviness happen. The character of the tissue has to be judged in person.
Cheek reduction, cheek bone surgery or lower cheek lipo — do you need them?
Patients regularly ask me about cheek bone reduction surgery or lower cheek lipo before we have even looked at a mirror together. We are a dermatology clinic: we do not perform cheek bone surgery or liposuction, and if your concern is genuinely the bone, that is a surgical consultation, not a filler one.
Much more often, a cheek bone line that looks too prominent is a contrast problem. The bone did not grow; the area under it emptied and the lower face became heavier, so the balance shifted.
When the lower face is what needs reducing rather than the cheek, the non-surgical tools are different from filler. Focused ultrasound lifting works on the deeper layer and the fat beneath it, botulinum toxin can reduce an over-developed jaw muscle and the salivary glands, and InMode Mini FX delivers pulsed energy at 45–47 °C to break down unwanted fat under the chin and improve jawline definition.

Technique is where bad cheek fillers are prevented, not rescued. Our dermal filler injections are done by dermatologists with over fifteen years of filler experience, and we use a cannula wherever possible because it minimizes the vessel-damage risk of a standard needle.

If we choose collagen-stimulating injections instead
Sculptra does not create volume by itself — the PLLA stimulates fibroblasts so your own collagen is produced, which is why the change appears gradually. Right after injection the area can look temporarily fuller from the injection fluid, and about a week later that settles before the real build-up begins.
Home care matters here. Massage the treated area five minutes, five times a day, for five days — if the particles are not distributed evenly, they can clump and form a nodule, which is the main thing to watch for with this treatment.

Do not add product on top to camouflage it. That is how a heavy mid-face becomes a heavier one.
Hyaluronic acid filler can be adjusted with a dissolving injection when the result feels excessive or is sitting in the wrong place, under the doctor's judgement after an in-person assessment. Once the area has settled, we can decide calmly whether it needs a smaller, better-placed volume, support at the temples, or a lifting device instead.
Cheek wrinkles are a skin-quality problem, not a volume one
Fine cheek wrinkles and crepey texture do not improve because a cheek is fuller — sometimes a stretched surface just shows them more clearly. For that layer we use polynucleotide skin boosters: Rejuran HB+ over four sessions at 2–3 week intervals, or Rejuran Tox, which pairs Rejuran HB+ 3 cc with micro-dosed skin botox 3 cc in one visit from ₩913,000 (VAT excluded).
And what about bad Thermage side effects?
People who have had bad cheek fillers often ask about bad Thermage side effects next, because the fear is the same: an irreversible change to the face. Two things drive most of that risk with energy devices — the tip, and the anaesthesia.
We use genuine Thermage and Ultherapy tips only, and if a tip were ever found not to be genuine we refund ten times the treatment price. We also do not use deep sedation for lifting: Air NOX keeps you awake, so you can see the tip and the shot count yourself, and you can still give feedback while energy is being delivered.
That feedback matters. Under sedation a patient cannot report anything, so energy tends to be pushed higher, which raises burn risk. Most of our patients are satisfied with 300–400 Ultherapy shots, and only about 2% need 600 or more.
Hollowing is the other worry, and it is a real one — strong energy can reach the fat layer and thin a cheek that was already thin. That is why depth and range are planned per face, and why Thermage MAX infuses AQ MAX before the RF pulses: in lab tests, AQ at 40 mM protected fibroblasts and kept collagen synthesis up under 43–55 °C heat. One honest note to finish. I care less about how much we can fill than about which face and which impression we are restoring, and that decision belongs in a consultation, not in a price list. Balance, harmony and looking like yourself are what make a face read young — not volume, and not aggressive lifting.

Not always. The tissue below the cheekbone is often strongly adherent, so filler alone may not fully correct the hollow. The degree of adhesion has to be judged in person, and in some faces a combination — support injections plus lifting, with lower-face volume reduced — gives a better balance than volume alone.
Do you perform cheek bone reduction surgery or lower cheek lipo?
