The result depends on the plan more than the device.
Ultherapy does not suit every face. On cheeks that are already thin it can work against you, and we say so first. If it does suit you, a board-certified dermatologist sets the depth, energy and shot count for your face. You stay awake, so you can check the tip and the count yourself.
Ultherapy alone or Prime FIT? Dr. Choi explains when each makes sense
Dr. Won Woo Choi, M.D., Ph.D., a board-certified dermatologist at WELLS, on what Fitting adds to Ultherapy, who it suits, how much it hurts and why you stay awake. Jump to the part you care about.
01
What Ultherapy alone can’t cover
It works on deep fat and the fascia. Dr. Choi says it can’t fully cover the upper fat and the loose skin above them.
02
What FIT adds
Fitting follows Ultherapy in the same visit. Dr. Choi uses it on the lower dermis and upper fat, so the loose skin is treated too.
03
Who FIT suits
Soft skin that lacks strength, or heavy sagging where reducing fat alone won’t fix the loose skin.
Ultherapy helps some faces and works against others
Dr. Ryu says it plainly: “Ultherapy isn’t right for everyone.” It tightens tissue and it can also reduce volume, so the shape of your face decides whether it helps.
01
Enough cheek, and a jawline losing its edge
Dr. Lee finds satisfaction is highest in people with moderate or plenty of cheek volume. Many patients in their 40s to 60s come for a sagging lower face.
02
One area that bothers you
Jowls, or a double chin on its own. Dr. Lee calls Ultherapy a very good choice when you want to treat only part of the face.
03
Soft skin on top of sagging
Skin that has lost its strength, or heavy volume where less fat alone would leave skin loose. This is who Dr. Choi recommends FIT for.
04
Thin or hollow cheeks
If your cheeks are already hollow and you want more volume, Dr. Ryu calls this the opposite treatment. With very little cheek the issue is often firmness and fine lines, and Dr. Lee would point you to Thermage.
Prime FIT is not a new machine. It is Ultherapy Prime followed by Fitting in the same visit. Fitting is how our doctors treat the loose skin that Ultherapy alone does not fully cover.

Depth and energy
Depth and energy shape the result more than the shot count
Different layers do different jobs. Aim at fat and volume falls; aim at the fascia and tissue tightens. Dr. Choi’s rule on energy: higher is not automatically better, and neither is lower. Tap each step to see where it lands.
EpidermisDermisFatSMAS
Drag to rotate
Ultrasound is focused on the fascia (SMAS) that holds up the fat above it. Dr. Lee describes this as pressing tissue down and fixing it in place, not hauling it upward. Dr. Choi usually picks this depth for fuller faces with a sagging jawline.
Dr. Lee chooses between 3 mm and 4.5 mm by cheek volume and the way the face sags. With little cheek, the aim is to refine the outline without taking volume.
Follows Ultherapy in the same visit. Dr. Choi uses it on the lower dermis and upper fat, for skin Ultherapy alone does not fully tighten. This is how our doctors use it, not a regulatory indication.
Ultherapy, then Fitting, in one visit. Dr. Choi’s summary of the difference: alone, you notice less volume. With FIT, less volume and tighter loose skin.
Tip depths are the manufacturer’s settings. Your dermatologist chooses the layer, depth and energy for each area of your face.
What can go wrong
Three things patients fear, and what we do about each
No doctor can remove these risks completely. Here is why each one happens and how we work to lower it.
01
Hollow cheeks
Why it happens
Fat sits right above the fascia. Too much energy there can shrink that fat, and the cheek looks emptier. Dr. Lee explains this as shrinkage, not fat being destroyed. Dr. Ryu names it as a typical side effect. A strong effect can also leave a lean face looking too thin.
What WELLS does
A dermatologist checks your face shape and skin before deciding. If your cheeks are already thin, we tell you Ultherapy may be the wrong choice and discuss Thermage. 3D analysis and the ultrasound view help plan the treatment, but they do not remove this risk.
02
Burns from poor tip contact
Why it happens
The energy is meant to land deep, so Dr. Lee says visible surface burns are uncommon. But if a gap opens between tip and skin, energy can enter near the surface and burn. Too little anatomy knowledge or experience can also put energy in the wrong place.
What WELLS does
A dermatologist treats you in person, with gel and the tip pressed flat against the skin, and uses the built-in imaging to check contact and depth. You are awake and can tell us at once if a spot feels wrong.
03
Sedation
Why it happens
At 400 to 600 shots, treatment can run 20 to 30 minutes. Dr. Choi’s view: brief sedation can be fine, but keeping someone under for about 30 minutes brings small risks that a cosmetic treatment does not need. Dr. Lee’s view: if you move without knowing it, tip contact can break, and that may raise burn risk.
What WELLS does
At WELLS, our doctors prefer to keep you awake with Air NOX, so you can tell us how each area feels during treatment and see the tip and shot count yourself. This is our clinic’s approach, not a manufacturer requirement. Pain is managed with numbing cream and local anesthetic injections.
Regulation and side effects
What it is cleared for, what can go wrong, who should not have it
Regulatory status differs by country, and it does not tell you whether this suits your face. Read this before you book.
US FDA
510(k)-cleared, for specific uses
The Ulthera® System, the platform behind Ultherapy PRIME®, is FDA 510(k)-cleared in the United States (PRIME control unit: K233996, Feb 22, 2024; latest indications: K260618, May 21, 2026). 510(k) clearance is not FDA approval. US-cleared uses: lifting the eyebrow; lifting lax tissue under the chin and on the neck; improving lines and wrinkles of the décolleté; improving the appearance of skin laxity on the abdomen, arms and knees. The US indications do not list the cheeks, the jawline or the whole face.
In Korea, Merz Aesthetics Korea states that Ultherapy PRIME is licensed by the Ministry of Food and Drug Safety as a focused ultrasound device for lifting the eyebrows, a sagging jawline and the upper chest, and improving upper-chest wrinkles.
Built-in DeepSEE® ultrasound imaging lets the doctor view skin layers up to 8 mm deep, to check transducer contact and confirm treatment depth, for example to avoid bone. It supports the doctor’s technique. It does not remove the risk of side effects.
The manufacturer restricts the system to properly trained physicians and trained persons under their supervision. At WELLS, a board-certified dermatologist consults, plans and treats you in person.
These statements apply to Ultherapy PRIME only, not to Fitting or the FIT package.

It can happen, and no doctor can promise it won’t. Too much energy in the fat layer can shrink that fat and leave the cheeks looking emptier. Dr. Lee explains this as shrinkage, not fat being destroyed. Your protection is a precise design of the area, a doctor who reads the ultrasound while treating, and a consultation that first asks whether your face suits this at all.
QMy face is thin. Should I still have it?+
Perhaps not. Dr. Lee finds satisfaction highest in people with moderate or plenty of cheek volume. With very little cheek the problem is often firmness and fine lines, and Thermage may suit you better. A careful plan can refine the outline of a lean face, but if you want more volume, Ultherapy is the opposite treatment.
QUltherapy Prime alone or FIT?+
Prime is Ultherapy alone. FIT adds Fitting in the same visit. Dr. Choi says a fuller, sagging face can respond to Ultherapy alone, which leaves you with less volume. FIT is for skin that has lost its strength, or heavy sagging where less fat would still leave loose skin. It does not last longer: Ultherapy sets the duration, and FIT changes the result.
QIs FIT more painful? How much does it hurt?+
It is not painless. With numbing cream and local injections, Dr. Choi says there is very little pain during Ultherapy, and Fitting adds some mild heat. Dr. Lee describes reducing pain by about 80% or more with a nerve block, so expect some to remain. Tell us if you are very sensitive.
QWhy won’t you put me to sleep?+
At 400 to 600 shots, treatment can run 20 to 30 minutes. Dr. Choi’s view is that keeping someone sedated for about 30 minutes brings small risks a cosmetic treatment does not need. Dr. Lee’s view is that moving unconsciously can break tip contact and may raise burn risk. Awake, you can tell us how each area feels. This is our clinic’s approach, not a manufacturer requirement.
QWhat do you use for pain, and who can’t have Air NOX?+
Numbing cream and local anesthetic injections, with Air NOX if you wish. Air NOX is oxygen and nitrous oxide; you stay conscious, adjust it with your breathing and use a single-use sterile filter and mouthpiece. It is not for early pregnancy, respiratory disease, panic disorder or vitamin B12 deficiency. Your care team explains preparation, recovery and driving. Ask at consultation whether local anesthetic injections cost extra.
QWhat about burns?+
Burns can happen. Because the energy lands deep, Dr. Lee says visible surface burns are uncommon. The main cause Dr. Lee describes is a gap between tip and skin, which sends energy toward the surface. That is why the gel, tip contact, the doctor’s anatomy knowledge and your being awake all matter. See the side-effect list on this page.
QI’m a foreigner. How would I know the tip is genuine?+
You can check. Dr. Lee warns that refilled and fake tips are widely reported, so ask about the tip and the machine at any clinic; the Ultherapy website explains how to check. At WELLS you are awake and can see the tip and the shot count, you can ask in English, and if a tip is not genuine we refund ten times the cost. Ask how the ten-times refund is calculated and claimed.
QHow many shots do I need? Is 300 enough?+
It depends on your face, so we set the count after 3D analysis. Our doctors describe a wide range: about 50 shots for a small area, under 300 for a lean face with little laxity, and 500 or more on the lower face alone with a heavy jawline. Dr. Choi says 300 can fall short for lifting the whole face. FIT 400 has 300 shots and FIT 500 has 400. Ask what a change would cost before treatment.
QHow long does it last, and why is it so expensive?+
Dr. Choi says it varies widely, from about 6 months to a year. Very saggy, soft skin tends to fade sooner, and firm skin with less sagging lasts longer. The cost comes largely from the tip, an expensive consumable. Value depends on the right depth and energy for your face, more than on the shot count or the price.
QHow soon will I feel normal? When can I fly?+
Dr. Ryu compares it to a bumped arm: sore and swollen for one to two weeks. About two weeks of soreness and firmness is normal. If you swell easily, tell us beforehand. We do not publish a rule for flying or for when you can return to activities, so give the dermatologist your dates and ask at consultation.
QDo I need Thermage too? Can I add something later?+
Not necessarily. Dr. Ryu names Thermage and Fitting as the two treatments that pair well with Ultherapy. Thermage heats the skin more broadly to help tighten it. If you want more a month or two later but not a repeat session, Fitting can be added. A dermatologist decides which, if any, after seeing you.
