How much ultherapy pain should you expect?

Hello, I'm Dr. Won Woo Choi, a board-certified dermatologist (M.D., Ph.D.) trained at Seoul National University Hospital and a Chief Director of WELLS Clinic in Apgujeong, Seoul. This article is based on my YouTube video from May 2026 above, where I explain ultherapy pain and why I don't use sleep sedation for it; translated captions are available on YouTube.

My answer: ultherapy pain can be kept low without putting you to sleep. Pain is a sensation carried by nerves, so numbing cream plus a few well-placed local anesthetic injections controls most of it. Air NOX can take the edge off further while you stay awake.

Staying awake is not a compromise. It is part of doing the treatment safely, and I explain why below, after a quick look at why Ultherapy works the way it does.

Why does Ultherapy work, and why can it sting?

Its current name is Ultherapy Prime. It focuses ultrasound energy about 4 mm below the skin, right at the fascia that supports the fat layer. The part of that fascia wrapping the face is called the SMAS, the same layer a surgical facelift tightens.

Because the energy tightens that deep structure point by point, it changes the lifting framework itself. That is why the effect is strong and lasts relatively long. It is also why you can feel it: the energy is concentrated deep in the tissue.

People often ask why it costs more than other devices. Put simply, the system is built so each patient uses a new consumable tip, and those tips are expensive. At WELLS we use genuine tips only, and you can see the tip and shot count yourself because you are awake.

Why don't we use sleep sedation for Ultherapy?

Some clinics offer sleep sedation. Once Ultherapy goes past 400, 500 or 600 shots, the treatment takes 20 to 30 minutes. Brief sedation for a short cosmetic procedure can be safe, but keeping someone sedated for up to 30 minutes brings risks that are small but real. Cosmetic treatment should put safety first.

There is a second reason. Ultherapy's own guidelines don't recommend deep sedation. When you are asleep you can't give feedback, so energy tends to be pushed higher, which raises the risk of burns. Awake, you can tell us when a spot is too strong and we adjust.

At WELLS we use numbing cream and local anesthetic injections, and if you are anxious, Air NOX, a mix of oxygen and nitrous oxide. It calms you while you stay conscious, and you control the intensity yourself by breathing. It is not suitable in early pregnancy, with respiratory conditions, panic disorder or vitamin B12 deficiency.

How WELLS treats it

Ultherapy Prime FIT

What it is
Ultherapy Prime plus a second 'Fitting' laser, so the deep SMAS layer and the layer above it are treated in the same visit.
How it works
Focused ultrasound reaches the SMAS layer, the same layer a surgical facelift tightens; the Fitting laser then works on the layer between Ultherapy and Thermage depth to tighten the surface contour along the jawline.
Sessions
One session; FIT 400 (300 shots + Fitting 100 kJ) or FIT 500 (400 shots + Fitting 100 kJ), recommended at consultation.
Recovery
No deep sedation. Numbing cream and local anesthetic, with Air NOX non-sedation if you wish; you stay awake and go home right after.
Price
FIT 400 ₩1,925,000 · FIT 500 ₩2,409,000 (VAT excluded)

Why patients fly in for it

  • A Merz Ultherapy Golden Record Award clinic, using the Ultherapy Prime system with genuine tips (10× refund guarantee).
  • Air NOX non-sedation: you stay awake and can see the tip and shot count.
  • Your face is analyzed with a 3D imaging system before energy and shots are mapped.

Is Ultherapy Prime FIT more painful than Ultherapy alone?

Not meaningfully. If Ultherapy itself is done with little pain, FIT is too. The only difference is that after Ultherapy we continue with the Fitting laser, and you may feel some heat during that part.

Ultherapy alone tightens the deep layer and reduces deep fat, but it cannot fully cover the upper fat layer and loose skin sitting above it. Prime FIT adds the Fitting laser for that layer, so the whole depth is lifted. I recommend it in two cases:

  • Skin that is soft and lacks strength, in someone who also wants lifting.
  • Heavy, sagging volume where reducing fat alone won't fix the loose skin.

On our Ultherapy Prime FIT page, FIT 400 (300 shots + Fitting 100 kJ) is ₩1,925,000 and FIT 500 (400 shots + Fitting 100 kJ) is ₩2,409,000 (VAT excluded).

How many shots, and how long do results last?

It depends on how much the face sags and how full the cheeks are. For a very small area, such as only the center of a double chin, even 100 shots can make a difference. For lifting the whole lower face, more is needed, and heavier, fuller faces need more again. Your dermatologist sets the count after a 3D analysis of your face.

How long it lasts varies a lot between people. Skin that sagged a lot and feels soft loses the effect sooner; firmer skin that sagged less keeps it longer. The range runs from about six months to a year.

Ultherapy itself decides the basic duration, with or without FIT. The difference is in the result: Ultherapy alone mostly reduces volume, while Prime FIT also tightens the loose skin. For what to watch for afterwards, see our post on Ultherapy side effects.

What does Ultherapy pain control mean for your trip?

Because there is no deep sedation, there is no fasting and no recovery room, and you go home right after. That makes it easier to fit into a short stay in Seoul. Send a photo and your travel dates on WhatsApp; we reply in English, and a dermatologist confirms the plan and shot count at consultation.

For price comparisons across options, see how much is Ultherapy in Korea. WELLS is about five minutes on foot from Apgujeong Station (Line 3), Exit 3. Results vary from person to person.

Q&A

Questions about this video

How is ultherapy pain controlled at WELLS?

With numbing cream and a few local anesthetic injections, plus Air NOX non-sedation if you wish. You stay awake, can give feedback, and go home right after.

Why not get sleep sedation for Ultherapy?

Larger treatments take 20 to 30 minutes, and keeping sedation that long adds small but real risks. Ultherapy's guidelines don't recommend deep sedation, and without your feedback energy tends to be pushed higher, raising burn risk.

Does Ultherapy Prime FIT hurt more?

No meaningful difference. You may feel some heat during the Fitting laser after the Ultherapy part.

How long does Ultherapy last?

It varies, from about six months to a year. Softer skin that sagged more tends to lose the effect sooner.

This article explains general medical information from the video. It is not a diagnosis; results vary by person and are decided in consultation.
