What does Thermage before and after really mean over time?

I'm Dr. Won Woo Choi, a board-certified dermatologist (M.D., Ph.D.) trained at Seoul National University Hospital and Medical Director of WELLS Clinic in Apgujeong, Seoul. This article is based on my YouTube video from April 2020 above, on the effect of Thermage FLX and how long it lasts; translated English captions are available on YouTube.

A Thermage before and after comparison is not one moment but a sequence: slight swelling and a glow at around two weeks, a tighter, held-in feeling at around one month, and then a softer feeling from about two months while new collagen keeps building underneath. The visible change is modest; the larger change is in how the skin works.

I made the video after reading a question that many patients ask me in some form: "My skin was plump for two weeks and felt lifted at one month, but at two months it feels like it's dropping again. I heard the real effect starts at three months. Is it already wearing off?" Below I explain what is happening at each stage, and why that feeling is not the treatment failing.

Why does the tight feeling fade at two months?

Because the swelling settles and the new collagen becomes part of the skin's structure. Thermage does not shine laser light. It passes an electric current through a set depth of the skin, and the current itself plus the heat created by the skin's resistance stimulate new collagen synthesis, to replace or add collagen in skin that has sagged and lost it.

That strong heat reaches from the middle to the lower dermis and causes a light, controlled injury there, and every injury comes with some swelling. That is why the skin is slightly puffy for about two weeks, which patients notice as a glow and plumpness.

As that injury recovers, new collagen is laid down. By about one month the swelling has mostly gone and part of the dermis has been replaced with new collagen, which is when the skin feels held in. As the new collagen settles in with the surrounding matrix, the skin becomes more stable and the "tight" sensation fades. When skin tissue is examined under a microscope after Thermage FLX, new collagen has been shown to increase and stay at three months, six months and a year. What has changed is the feeling.

How does Thermage MAX at WELLS support that collagen?

Our Thermage Korea protocol, Thermage MAX, is built around the collagen-producing cells the heat depends on. It is Thermage FLX with three additions:

  • Skin-microbiome test: a sample is analyzed for oil and moisture levels, microbial diversity, barrier health and vulnerability to inflammation, and repeated when you return a year later.
  • AQ MAX infusion, two bottles: alanyl-glutamine is infused before treatment so fibroblasts survive the heat of the RF pulses. In lab tests, AQ (40 mM) protected fibroblasts and kept collagen synthesis up under 43–55 °C heat.
  • Collagen booster serum: applied as the FLX handpiece works, it adjusts the skin's conductivity to reduce the sting while its collagen peptides support collagen synthesis.

It suits loose skin on the cheeks, jawline and neck, fine lines and loss of firmness, and anyone who has had standard Thermage and wants a more consistent result. Your dermatologist decides at consultation whether 600 or 900 shots fits the area to be treated.

How WELLS treats it

Thermage MAX

What it is
Thermage FLX with a skin-microbiome test, AQ MAX infusion and a collagen-peptide serum, developed as part of the WELLS Thermage MAX protocol.
How it works
Monopolar radiofrequency heats the dermis to stimulate new collagen; two bottles of AQ MAX (alanyl-glutamine) are infused beforehand so collagen-producing fibroblasts are protected from the heat, and a collagen-peptide serum is applied as the handpiece works.
Sessions
One session; Dr. Choi suggests repeating about once a year, when the microbiome test is also repeated and compared.
Recovery
Mild swelling for around two weeks is common; details confirmed at consultation. No deep sedation, and you can drive afterwards.
Price
600 shots ₩3,465,000 · 900 shots ₩4,477,000 (VAT excluded)

Thermage before and after: what to expect at each stage

  • Around two weeks: the skin is slightly swollen, so it looks plumper and has more glow.
  • Around one month: the swelling has mostly settled and the skin feels held in and tighter.
  • From about two months: the tight sensation softens as new collagen stabilises; the collagen itself keeps building and staying.
  • Over the year: better moisture-holding and, in my view, a slower rate at which fine lines form.

Patients sometimes say, "It was an expensive treatment and the visible change is small." That is fair, so think of it this way: every year collagen decreases and what remains ages and loses strength. If Thermage replaces part of it with fresher collagen, the visible change at two months may be modest, but the fine lines you might have had a year later can form more slowly, and the skin's moisture function stays improved through the year.

How long does it last? It comes down to the published data, which suggest the new collagen from one session stays for about a year to a year and a half on average before it gradually loses function. That is why I suggest repeating Thermage about once a year. For comfort, we do not use deep sedation: non-sedation Air NOX with local anesthetic keeps you awake, no fasting is needed, and you can drive afterwards.

Why do patients fly to WELLS for Thermage?

Because they can see what they are paying for, and measure it. At WELLS a board-certified dermatologist performs your Thermage in person, and Thermage MAX adds a microbiome test you repeat a year later, so your own comparison is based on measurement rather than memory.

  • Genuine Thermage FLX tips; if a tip is ever found not to be genuine, we refund ten times the treatment price.
  • Dr. Won Sin Lee lectured at Thermage's headquarters in 2016, and the clinic exchanges Thermage FLX cases with a New York laser dermatology practice.
  • No unnecessary sedation, no recovery room, and consultations available in English.
  • In Apgujeong since 2008, five minutes from Apgujeong Station Exit 3.

How do you plan a Thermage trip to Seoul?

Plan one visit and allow for about two weeks of mild swelling around your travel dates; then plan your return about a year later. Thermage MAX is listed at ₩3,465,000 (VAT excluded) for 600 shots and ₩4,477,000 (VAT excluded) for 900 shots.

Rather than looking at other people's results, send us a recent photo of your own face, your concerns and your travel dates on WhatsApp. A dermatologist will tell you what change is realistic for your skin and which shot count fits. Before you book, my posts on thermage side effects and thermage cost korea cover the rest of a Thermage before and after decision.

Results vary from person to person, and the right plan for you is decided in consultation with your dermatologist.

Q&A

Questions about this video

When will I see my Thermage before and after difference?

Around two weeks the skin looks plumper because of slight swelling; around one month it feels tighter. From about two months the tight feeling softens while new collagen keeps building. The visible change is modest and varies by person.

Is it normal for the tight feeling to fade after two months?

Yes. The swelling has settled and the new collagen has stabilised in the skin. Tissue studies show new collagen staying at three, six and twelve months, so a fading feeling is not a fading effect.

How long does Thermage FLX last?

Published data suggest the new collagen from one session stays about a year to a year and a half on average, which is why Dr. Choi suggests repeating it about once a year.

Does WELLS show patient photos?

No. Instead, send a recent photo of your own face on WhatsApp and a dermatologist will tell you what change is realistic for your skin.

How much is Thermage MAX at WELLS?

₩3,465,000 (VAT excluded) for 600 shots and ₩4,477,000 (VAT excluded) for 900 shots. Your dermatologist confirms the right option at consultation.

This article explains general medical information from the video. It is not a diagnosis; results vary by person and are decided in consultation.
