Korean Laser Treatment for Melasma: Why It Returns and How Pico Toning Helps
Why melasma keeps coming back, which pico wavelengths matter, how many sessions to expect and what side effects to know about before you book in Seoul.
Can a Korean laser treatment really fade melasma?
I'm Dr. Won Woo Choi, a board-certified dermatologist (M.D., Ph.D.) trained at Seoul National University Hospital and Medical Director of WELLS Clinic in Apgujeong, Seoul. This article is based on my YouTube video from December 2021 above, about pico toning for melasma and spots; translated English captions are available on YouTube.
Yes, a Korean laser treatment such as pico toning can fade melasma, but it does so gradually over a series of sessions, and melasma is managed over time rather than cleared in one trip. If your pigment came back after laser toning before, the answer is usually not "more of the same laser" but a different wavelength plan and, for recurring pigment, treating the skin around the pigment too.
Below I explain why melasma returns, how we treat it with two picosecond wavelengths, how many sessions to expect and what side effects to know about.
Why does melasma keep coming back?
Melasma is not caused by sun exposure alone. Aging by itself can make it spread and darken, which is why people notice it even in years when they spend little time outdoors. Recent studies also point to senescent fibroblasts, collagen loss and a weakened basement membrane in melasma skin, so the environment the pigment sits in matters, not just the pigment.
The laser itself is the second reason. For years pigment was treated with conventional laser toning; picosecond lasers then became the mainstream choice because they break pigment into finer particles with less heat. But "pico toning" is a name, not a device. Picosecond lasers fall into two main groups, 1064 nm and 755 nm, and in my experience neither wavelength alone clears melasma well, because each has its own strengths and limits.
So when pigment returns, it is worth asking which wavelength was used, at what interval, and whether anything was done for the skin underneath.
How does WELLS treat melasma with pico laser toning?
We use both wavelengths. At WELLS, pico toning is done at 1064 nm and 755 nm, and PicoSure Pro, a 755 nm picosecond laser, is on our menu. The pulses are about 1,000 times shorter than conventional laser toning, delivering strong peak power that breaks abnormal pigment into finer particles with less heat to the surrounding skin.
It treats melasma, age spots (lentigo), nevus of Ota and ABNOM. It can also treat dark circles caused by pigment, though not every dark circle: some come from blood vessels showing through thin skin, or from hollows, and those will not respond. On the body, brown and dark pigment and large café-au-lait spots usually need the two wavelengths combined.
For melasma that keeps returning after toning, our MELA Triple Toning protocol adds Potenza microneedle RF and a skin-brightening enzyme developed at WELLS with LDM ultrasound, and is listed at ₩770,000 (VAT excluded) per session. You can read how we approach pigment on our melasma pico toning page.
Melasma & Stubborn Pigment (Pico Toning)
- What it is
- Picosecond laser toning at 1064 nm and 755 nm for melasma, age spots (lentigo), nevus of Ota and ABNOM.
- How it works
- Picosecond pulses deliver strong peak power in a very short pulse, breaking abnormal pigment into finer particles with less heat to the surrounding skin; the two wavelengths are combined to reach pigment at different depths.
- Sessions
- A series, set at consultation; in Dr. Choi's experience around ten sessions is a realistic plan, usually every two weeks.
- Recovery
- Confirmed at consultation; the interval is adjusted to how red your skin becomes after each session.
- Price
- Confirmed at consultation
Why patients fly in for it
- Both 1064 nm and 755 nm picosecond lasers, including PicoSure Pro.
- Dr. Choi presented a five-session analysis of combined pico laser melasma treatment at the Korean Dermatological Association meeting.
- A board-certified dermatologist adjusts energy, sessions and interval at every visit.

Message on WhatsAppHow many sessions does a Korean laser treatment for melasma take?
It takes several sessions; one or two will not clear melasma or spots. Deep, widespread melasma needs more, and light, small areas need fewer. As a rough guide from my experience, many people find around ten sessions a realistic plan. How far pigment fades differs from person to person, so this is a planning range, not a promise.
The interval depends on how red your skin becomes after treatment and how long that lasts. It ranges from every one to two weeks to about once a month; every two weeks is the most common rhythm.
Pico toning generally causes fewer side effects than older laser toning, because pigment clears with less energy, but it is not free of them. Known side effects include mild temporary skin reactions and, with long courses or short intervals, white spots where pigment has been lost. In my experience these are uncommon. The more useful question is whether you see a reasonable response within the early sessions, which is why the number of sessions and interval can change as treatment goes on.
Why do patients fly to WELLS for pico toning?
Because pigment is one of my own clinical fields. I presented a five-session analysis of combined pico laser melasma treatment at the Korean Dermatological Association meeting, and WELLS has used PicoSure since introducing it in 2015.
- Both 1064 nm and 755 nm picosecond lasers under one roof, so your plan is not limited to one wavelength.
- A board-certified dermatologist handles your consultation, diagnosis and treatment, and reviews your response at each visit.
- A protocol for recurring melasma that treats the skin environment as well as the pigment.
- Consultations are available in English, and the clinic has been in Apgujeong since 2008.
How should I plan a Korean laser treatment trip to Seoul?
Plan for continuity with one doctor. Melasma treatment is rarely finished in a single short trip, and energy, sessions and interval are adjusted to your response, so it helps to stay with one clinic and review results at each visit. Send a photo and your travel dates on WhatsApp, and we will suggest how sessions could fit your stays.
The price for pico toning at WELLS is confirmed at your consultation. For more background, see my posts on pico toning and korean melasma treatment.
Results vary from person to person, and the right plan for you is decided in consultation with your dermatologist.
