Laser Toning Side Effects: Why White Spots Form and How We Avoid Them
Why melasma returns after toning, why white spots can appear after many sessions, how they recover, and how WELLS plans melasma care to use less laser.
What are the main laser toning side effects?
Hello, I'm Dr. Won Woo Choi, a board-certified dermatologist (M.D., Ph.D.) trained at Seoul National University Hospital and Chief Director of WELLS Clinic in Apgujeong, Seoul. This article is based on my YouTube video from July 2026 above, the second part of my series on pigment lasers.
Two laser toning side effects come up again and again in my clinic: melasma that comes back after it has faded, and small white spots that appear after a long course of toning. The first is the nature of melasma. The second is a real side effect, and it can be made less likely with the right laser and the right plan.
Below I explain why each happens, what to do if you already have white spots, and how we plan melasma treatment at WELLS so that it relies less on laser.
Why does melasma come back after toning?
Is melasma bad for your skin? Not exactly. Melasma forms where skin is thin or vulnerable to UV, and the skin produces melanin there as a dark curtain, so UV cannot get deep inside. It is a natural defence.
That means we cannot erase it for good. The aim of laser treatment is to lighten melasma until it no longer bothers you, not to remove it. If you get a lot of sun again, or as time passes, it slowly rises again. This is not a failure of the laser; it is how melasma behaves.
Why do white spots appear after many sessions?
Laser toning has been used in Korea for about 20 years, from Q-switched Nd:YAG lasers to picosecond lasers. Its method is low energy, repeated many times. Each pass is meant to fade pigment gently, but the laser's effect on melanin is not perfectly even every time.
In one session melanin may break up a lot, in another only a little. Over many repeats, that variability can damage some pigment cells, and those spots stop making colour, leaving white patches. This is why white spots are seen as a side effect of long, repeated toning.
With repeated Q-switched Nd:YAG toning, white spots appeared more often than expected. Picosecond lasers were developed to remove pigment more selectively and more safely, and with pico toning white spots occur less often, even over repeated sessions. At WELLS we use pico toning at 1064 nm and 755 nm for melasma.
MELA Triple Toning
- What it is
- A WELLS melasma protocol in three steps: Pico or Clarity toning, Potenza microneedle RF, and a skin-brightening enzyme developed at WELLS applied with LDM ultrasound.
- How it works
- Toning breaks pigment into fine particles, Potenza works on the deeper skin environment, and the enzyme helps fade melanin, so the plan does not rely on laser alone.
- Sessions
- The listed price is per session. Melasma is managed over time; the dermatologist sets the number and interval at consultation.
- Recovery
- Confirmed at consultation.
- Price
- ₩770,000 per session (VAT excluded)
Why patients fly in for it
- A board-certified dermatologist diagnoses your pigment before choosing energy, wavelength and number of sessions.
- Picosecond toning at 1064 nm and 755 nm, rather than relying on long courses of older Q-switched toning.
- Laser combined with an in-house brightening enzyme, so melasma can be treated with less laser.

Message on WhatsAppCan white spots from laser toning be treated?
Yes, slowly. When white spots appear during melasma treatment, we stop the laser for a while or switch to non-laser melasma care. Given rest, colour gradually fills back into the white spots.
It takes a long time: about three to six months before the spots fill in enough to be much less noticeable, and they may not return completely to your normal skin colour. There is honestly no treatment that removes them on the spot, even if your work keeps your face on camera.
Every dermatologist treats melasma carefully because of this. Even so, white spots can appear unexpectedly, one or two or a small cluster, because skin varies. When they do, the answer is to lower the intensity or pause, and give it time.
How does WELLS lower laser toning side effects?
Melasma treatment takes many sessions, sometimes over years, so experience matters. I have been treating melasma for close to 20 years. With that, when a patient first comes in, I can judge which energy range suits that melasma, or decide that this person should do five sessions and then rest.
We also do not rely on laser alone. Today there are other ways to fade melasma: oral melasma medicine or supplements that reduce pigment over a short period, and enzyme treatment that breaks down melanin alongside the laser. At WELLS we combine laser with an enzyme that fades melasma and, when it fits, oral medicine.
That is the idea behind MELA Triple Toning: toning, Potenza microneedle RF for the deeper skin, and a brightening enzyme developed at WELLS with LDM, ₩770,000 (VAT excluded) per session. Using less laser while still reaching clear skin is the goal.
How should I plan melasma care in Seoul if I've had toning before?
Tell us what you have had: how many toning sessions, roughly which laser, how often, and when you stopped. Send a daylight photo without makeup and your travel dates on WhatsApp, and the dermatologist will tell you whether to treat now, rest first, or change approach.
If you are still deciding which laser your spots need, start with my first video on Korean dark spot treatment, which matches each type of spot to the right laser. Laser toning side effects are one reason diagnosis comes before any laser.
WELLS is on the 8th floor of the Apgujeong Building, about five minutes on foot from Apgujeong Station (Line 3), Exit 3. Results vary from person to person, and the plan is decided with your dermatologist.

Message on WhatsAppQuestions about this video
What laser toning side effects should I know about?
The main one Dr. Choi describes is white spots (loss of pigment) after long, repeated toning. Melasma returning over time is not a side effect but the nature of melasma.
Do white spots after toning go away?
After pausing or lowering the intensity, colour usually fills back in gradually. It takes about three to six months to become much less noticeable, and it may not fully match your normal skin.
Is pico toning safer than older toning?
Picosecond lasers were developed to remove pigment more selectively. With pico toning, white spots occur less often than with repeated Q-switched toning, though no laser removes the risk entirely.
How much is MELA Triple Toning at WELLS?
₩770,000 (VAT excluded) per session. The number of sessions is set by your dermatologist.
This article explains general medical information from the video. It is not a diagnosis; results vary by person and are decided in consultation.
