Freckles and melasma can look alike, but they sit at different depths and react to laser differently. Here's how we tell them apart and treat each at WELLS.

Freckles vs melasma: why the difference matters
Brown spots on the cheeks all look like the same problem in the mirror. But freckles vs melasma is one of the most important distinctions in pigment treatment, because a laser setting that clears freckles quickly can make melasma darker.
I'm Dr. Won Woo Choi, a board-certified dermatologist with a Ph.D. in dermatology from Seoul National University and one of the chief directors at WELLS in Apgujeong. Pigment has been treated at WELLS since 2008, and the first thing I do with any spot is work out what it is before deciding how to treat it.
This post explains how the two differ, why they need opposite approaches, and what treatment for each looks like at our clinic. Diagnosis comes before the laser, every time.
How do you tell freckles and melasma apart?
Neither is a reliable self-diagnosis, and many faces have both. Still, there are general patterns a dermatologist looks for.

Sun spots and lentigines (age spots) sit in a third group. They are also driven by UV and often behave more like freckles, but larger or thicker ones need to be reduced gradually rather than removed in one go.
Why melasma behaves differently in the skin
Melasma is not simply extra pigment piled up in one spot. Long-term UV exposure and hormonal influence change the environment of the dermis itself. Recent studies point to senescent fibroblasts and a weakened basement membrane in melasma skin, which is why we treat the skin environment and not only the pigment.
Melasma skin also tends to have a weaker barrier and dilated blood vessels. Firing strong energy at it can stimulate the surrounding pigment cells, and the result can be rebound darkening, white spots where pigment cells were damaged, or blotchy fading that looks worse than before.
Freckles are the opposite case. The pigment is closer to the surface, not densely packed and clearly bordered, so appropriate energy tends to show an early brightening change.
How we treat freckles at WELLS
For freckles and sun spots we use BBL at 515 nm with strong cooling. BBL keeps the wavelength best suited to pigment uniform from the first pulse to the last, so light freckles that IPL leaves behind can be cleared.
Its cooling system makes 515 nm safe for Asian skin, which carries melanin even in normal areas. We recommend it if IPL left faint freckles, if you want the result of 2–3 IPL sessions at once, or if you worry about IPL side effects.
In general, with IPL or pico laser, more than 70% of a freckle can be expected to shed after a single session. Fine crusts form and fall off as the tone brightens, and skin typically clears by around day seven. Freckles return easily with UV, so sun protection keeps the result.

For melasma the rule is gentle and consistent, not strong. We use picosecond toning at 1064 nm and 755 nm, with pulses 1,000 times shorter than laser toning. The strong peak power in a very short pulse breaks pigment into finer particles with less heat to the surrounding skin.
At a low setting the irritation stays down, and the fragmented pigment is gradually cleared by the body through the lymphatic system. I presented a five-session analysis of combined pico laser melasma treatment at the Korean Dermatological Association meeting, showing results more than 40% faster than conventional toning: about 5–7 picosecond sessions compared with 10 conventional laser-toning sessions.
For melasma that keeps coming back after toning, we developed MELA Triple Toning. It combines pico or Clarity toning for the pigment, Potenza microneedle RF to renew the deeper skin environment, and a skin-brightening enzyme developed at WELLS applied with LDM ultrasound. It is listed at ₩770,000 (VAT excluded) per session.
Melasma is managed over time. The dermatologist sets the number of sessions and the interval at consultation, based on how deep the pigment sits and how your skin responds. Our PicoSure Pro, a 755 nm picosecond laser, is another option for pigment at different depths.

What if you have both freckles and melasma?
This is common, and it is where the order of treatment matters. If melasma is present, I plan around it first, because the settings that suit freckles can irritate melasma nearby.
In practice that often means settling the melasma with gentle toning first, then treating the clearer, well-defined freckles once the skin is calm. The dermatologist decides the sequence at consultation, after looking at the spots in person rather than in a photo.
Laser type, energy and interval all change with the pigment's characteristics. Precise diagnosis decides the result more than the device name does. That is also why I'm cautious when people ask for a single best Korean melasma treatment: what works depends on the depth, the cause and how the skin has reacted before.
Small, faint lentigines often clear in one or two focused sessions. Large ones, or those on thin skin around the eyes, are reduced gradually to avoid scarring. If you're searching for melasma treatment in Korea because toning elsewhere didn't hold, tell us what you had and when.
Aftercare and planning a pigment visit from abroad
- Apply sunscreen before going out. UV on treated skin can leave pigment darker or create new spots.
- Moisturize well, and avoid alcohol, saunas and anything that raises body temperature for 3–5 days.
- Let crusts fall off on their own. Picking them can leave marks.
- Skin may feel sensitive for a few days as the barrier recovers; we often add LDM ultrasound soothing after laser.
Freckles fit a short trip well, because the change shows early. Melasma is different: for most people it is a course of sessions, so tell us how often you can be in Seoul and the dermatologist will plan around it. Pigment treatment often works at intervals of 2–4 weeks, adjusted to your skin.
