Sunken Face After Weight Loss: Fill What Hollowed, Lift What Sagged

Why do you get a sunken face after weight loss?

Weight-loss injections have become very popular, and more and more people come to me with a sunken face after weight loss. This article is based on my YouTube video from February 2026 above.

The reason is simple: when facial fat disappears quickly, the face hollows and sags at the same time, and both are the direction of ageing. These treatments work well, so the change can be sudden, and the face can look older than the body.

The good news is that a sunken face after weight loss can usually be improved without surgery, as long as you treat the hollowing and the sagging as two separate problems.

Flat face or peanut face: which one is yours?

I see two common patterns. In a flat face, the front of the cheeks loses volume and sags, so the mid-face looks flat. The mid-cheek groove becomes visible and the nasolabial folds deepen.

In a peanut-shaped face, someone who already had strong cheekbones loses side-cheek fat. The cheekbones and jaw corners stay prominent while the area between them dips, so the outline pinches in the middle.

The two need different treatment, and so does your preference: do you want an immediate change, or a gradual, natural one? The answer changes the plan completely.

How do you fill a sunken face after weight loss?

What has hollowed needs volume back. There are three broad ways: filler, fat grafting (a surgical option), or Sculptra, which slowly rebuilds volume with your own collagen.

• Filler lifts the front cheek immediately. Its known risk is injection into a vessel, which can damage skin, so many people now prefer other options. At WELLS, dermal fillers are injected with a cannula where possible.

Side-cheek hollows also need filling, but the cheekbone area often has bone close beneath and tissue adhesions below it. Even with filler, some people can't be fully lifted there; how much can be restored depends on the individual.

How do you tighten what sagged?

Hollow areas need filling, and sagging areas need tightening; only then does it read as a lift. For the lower face I use Ultherapy Prime to reduce heavy fat and tighten the deep layer, and a contour injection to slim the jaw-corner muscle and salivary gland. Together they smooth the lower face while reducing its bulk, without surgery.

So the middle of the face is filled with filler or Sculptra, and the sagging lower face is slimmed and tightened. If you want more lift still, a thread lift is the next step.

People who keep up regular tightening and lifting, such as Thermage or Onda, tend to notice this problem less when they lose weight. People who have never had anti-ageing care usually feel it more.

A sunken face after weight loss is several signs of ageing at once, so one treatment rarely fixes it. A combination of lifting, filler, Sculptra and collagen boosters usually gives the result you want.
