PLLA filler explained: how Sculptra builds volume, and who it suits
PLLA filler doesn't fill, it makes your skin build collagen. How Sculptra differs from HA filler, what happens week by week, and how we avoid nodules.
What is PLLA filler, and is it really a filler?
PLLA filler is the name people search for Sculptra, an injectable made of poly-L-lactic acid (PLLA). The name is a little misleading: PLLA does not fill a hollow the way a gel does. It stimulates the fibroblasts around it so your skin makes its own collagen, and the volume arrives gradually.

PLLA is a plant-derived synthetic polymer. Injected as a fine powder suspended in liquid, it acts as a scaffold in the skin and then breaks down into water and carbon dioxide, while the collagen it prompted stays. Sculptra holds both US FDA and Korean MFDS approval.
The analogy I use in consultation: hyaluronic acid filler is buying flowers and putting them in a vase. Sculptra is planting a seed and letting the flower grow.
PLLA filler vs hyaluronic acid filler: what's the difference?
Both improve volume, but they solve different problems. HA filler is strong at shaping a specific contour; PLLA is strong at restoring the overall thickness and volume of the skin.

Because PLLA volume is your own tissue, it breaks down more slowly than an external substance, and it moves with your expressions. When a whole face has slowly deflated and looks tired, I often find Sculptra the more natural choice. When one area needs a clear shape, such as a forehead filler or the chin tip, our dermal fillers are usually the better tool.
What happens after a PLLA injection, week by week
Right after treatment the area looks fuller, but that is the injection fluid, not the result. Within about a week the fluid is absorbed and the face looks much as it did before, which is exactly when people panic unnecessarily.
From there the PLLA slowly breaks down and collagen production begins, and from about four to six weeks your own tissue starts to fill in. Because the final volume shows at around three to six months, the dermatologist has to predict it at the first session, which is why this treatment needs experience with facial structure.
We generally recommend about 3 sessions, one month apart, building collagen step by step rather than all at once. That pacing is also what keeps the result from looking done.
Why cc is the wrong question
Patients often ask how many cc they will get. With Sculptra, the PLLA content matters more than the volume of liquid: dilute a vial more and you inject more cc at a lower concentration, like adding water to coffee. What counts is how the PLLA in one vial is used, where, and by what criteria.
Filler in temples to lift eyes, filler in cheeks to lift jowls: where PLLA works
PLLA works best where fat loss is noticeable: the temples, the front cheeks and the upper part of the nasolabial folds. When ageing hollows the side cheek below the cheekbone, it can also soften that edge so the face shape reads smoother.
The searches for filler in cheeks to lift jowls or filler in temples to lift eyes point at a real idea, with a catch. Filling the area that sags tends to look heavy. Mobile areas such as around the mouth and eyes carry a higher risk of nodules, so dose and depth there have to be adjusted extremely finely. Under the eyes, where skin is at its thinnest, particle-based collagen stimulators can rarely leave a palpable nodule, and I take a very conservative approach.
PLLA also isn't a quick fix. If you need a visible change for an event next week, it is the wrong treatment, and I'll say so at consultation.

How we prevent nodules with PLLA
A nodule is where injected particles clump in one spot and feel hard. It is the main thing to watch with Sculptra, and it is largely preventable with technique and aftercare.
- Proper hydration time. We never mix the powder hastily just before treatment; it is given the full time to disperse evenly.
- The right depth, by micro-cannula. Too shallow raises nodule risk and too deep weakens the effect.
- Dose adjusted by area, especially in mobile zones.
- Reassessment every session. Where the treatment calls for it, 3D analysis shows volume loss and asymmetry the eye misses, and we adjust the amount and areas each time.
Your part is the 5-5-5 rule: massage the treated area for 5 minutes, 5 times a day, for 5 days after each session. We show you exactly where before you leave, because skipping it makes clumping more likely.

The price of a standard Sculptra treatment is confirmed at consultation; Sculptra Lifting is listed at ₩1,320,000 (VAT excluded) per session. The dermatologist tells you at consultation whether one session is enough or a course is needed.
