Korean melasma treatment: why we go gentle, and how to plan a visit
How a Seoul dermatologist approaches melasma: diagnosis first, gentle repeated toning, a protocol for pigment that keeps returning, and trip planning.

Why does melasma keep coming back?
If you are reading about Korean melasma treatment, you have probably had the patch across your cheekbones lighten and then return. Creams did not shift it, and a strong laser somewhere may even have made it darker. That is a common story, and it is not your fault.
I'm Dr. Won Woo Choi, a board-certified dermatologist trained at Seoul National University Hospital and one of the chief directors at WELLS. Of all pigment problems, melasma is the one I explain most carefully, because the instinct to treat it hard is exactly what goes wrong.
Melasma is not just extra pigment sitting on the surface. It is driven by UV, hormones, genetics and extra blood vessels in the skin, and the pigment often reaches down into the dermis. Melasma is managed over time, not erased in one visit.
Freckles vs melasma: why diagnosis comes before any laser
Spots that look alike can sit at different depths and have different causes. Freckles and most sun spots are close to the surface and usually respond early to the right energy. Melasma is deeper and more reactive.
Many faces have both, plus age spots or a birthmark such as nevus of Ota. That is why the first step of any melasma treatment in Korea should be a dermatologist deciding what each mark actually is. Research on melasma also points to the skin around the pigment: ageing fibroblasts, collagen loss, a weakened basement membrane and often a fragile barrier with dilated vessels. That changes how I treat it.
Waiting it out rarely helps. Once melasma and sun spots appear they seldom fade on their own, and the longer they are left, the more they tend to spread into the surrounding skin. An early, accurate diagnosis gives you the widest choice of gentle options.
What makes a Korean treatment for melasma different: gentle and consistent
The key to melasma is not strong, it is gentle and consistent. Too much energy can cause rebound darkening, white spots where pigment cells were destroyed, or blotchy fading that looks worse than before. Repeated gentle sessions are safer than one aggressive one.
When people ask me what is distinctive about Korean melasma treatment, this is my answer: patience in the settings. Low-fluence toning reduces irritation while the fragmented pigment is cleared gradually by the body, instead of being burned off at once.
Our base treatment is pico toning for melasma and stubborn pigment, at 1064 nm and 755 nm. The pulses are 1,000 times shorter than conventional laser toning, so pigment breaks into finer particles with less heat reaching the surrounding skin.
I presented a five-session analysis of combined pico laser melasma treatment at the Korean Dermatological Association meeting. It showed results more than 40% faster than conventional toning, about 5–7 picosecond sessions compared with 10 conventional toning sessions. The price for pico toning is confirmed at consultation.

Is there a Korean melasma peel?
People often search for a Korean melasma peel. Chemical peels are not on our listed treatments, so I would rather be clear than send you looking for one here.
What we do instead is aim for what people hope a peel will give them, a brighter and more even tone, using gentle picosecond toning and, for returning melasma, the three-step MELA protocol. LDM ultrasound after the laser also helps calm redness and dryness.
What to expect after a melasma session
Right after pigment laser, the skin can look a little red, feel dry and sting. That is a small inflammatory reaction where the epidermis meets the dermis, and it is part of normal recovery. We often do LDM soothing care straight after the laser to calm it.
Some people say their skin feels thinner for a few days. Most pigment lasers do not actually change skin thickness; the barrier is simply more sensitive while it recovers.
Timing between sessions matters in both directions. Too long a gap lets new pigment cover the progress; too short a gap irritates skin that has not recovered. That is why the dermatologist sets your interval rather than a fixed calendar.
What you can do at home
- Daily broad-spectrum sunscreen, reapplied when you are outdoors.
- A plain, generous moisturiser while the skin feels dry.
- No scrubbing or picking at fine crusts; they fall off naturally.
- Keep your usual routine simple until the dermatologist says otherwise.
Good home care will not replace treatment, but poor home care can undo it. Of everything in Korean melasma treatment, sun protection after each session is the part patients most often underestimate.
Planning melasma treatment in Korea around your trip
Because melasma is treated in a series, the realistic goal for a short trip is a proper diagnosis, a first session and a clear plan for what comes next. Sessions are usually spaced a week or more apart, and the dermatologist sets the interval for your pigment depth and skin.
