Forehead filler: what it can reshape, and when it's the wrong tool

What can forehead filler actually change?

If your forehead looks flat, dips in the middle or has a hollow just above the brows, you have probably wondered whether forehead filler could smooth that curve. It often can. What it cannot do is make a forehead shorter or narrower, and that difference decides whether filler is the right tool for you.

I'm Dr. Won Woo Choi, a board-certified dermatologist trained at Seoul National University Hospital and one of the chief directors at WELLS in Apgujeong. In this post I'll explain what forehead filler is for, which forehead concerns need something else, and how we plan it when someone flies in to see us.

In short: filler adds volume. It softens a sunken or uneven forehead into a gentle, rounded contour. Lines that appear when you raise your eyebrows, a brow that has dropped, or a hairline that sits high are different problems with different answers.

Flat, lined or heavy: which forehead problem do you have?

Most people who ask me about forehead fillers are describing one of three things. Working out which one comes first, because the treatment follows the cause, not the area.

In real faces these overlap. A forehead can be slightly flat and also crease when you talk. That is why the plan is made in person, with the face moving, rather than from a single photo.

How we inject forehead fillers at WELLS

Forehead filler at WELLS is planned and injected by a dermatologist. We inject with a cannula where possible, because a blunt cannula minimizes the vessel-damage risk that comes with a standard sharp needle.

The goal is balance. Forehead volume is shaped so the face does not look larger, which is the usual worry with an overfilled, bulging forehead. The contour should look like your own forehead on a good day, not a new structure.

The fillers generally used for this are hyaluronic acid, based on a substance that already exists in the skin. If a hyaluronic acid result ever needs adjusting, it can be dissolved with a dissolving injection when the doctor judges it appropriate. Forehead wrinkles when raising eyebrows: filler or botox?

Horizontal lines that appear when you raise your eyebrows come from the forehead muscle folding the skin. Filler does not stop that movement, so it is rarely the first answer for these lines.

Botulinum toxin relaxes the muscle so the skin is folded less. At WELLS we use botulinum toxin with US FDA approval only, and genuine product and quantity are guaranteed. Toxin does not regenerate the skin itself; it eases the folding that etches the lines in.

Where a forehead is both flat and lined, the two work together. Combining filler with botox extends the result, because less movement means less stress on the shaped area. If you're weighing toxin brands and dosing, ask about that at consultation; it's a separate decision from the filler itself.

How to make my forehead smaller: surgery, hairline and lifting

A lot of people searching for forehead filler are really asking how to make a forehead look smaller. I want to be straight about this: filler adds volume, so it cannot shorten a forehead. On a very high forehead, filling it can even draw more attention to it.

Forehead reduction surgery, a surgical forehead lift and hairline lowering with a hair transplant are surgical procedures. WELLS is a dermatology clinic and does not perform them, so if forehead reduction in Korea is what you're after, that is a question for a surgical specialist.

What we can look at is whether something else is making the forehead read as large. Is forehead filler safe? What to ask any injector

Filler restores volume immediately, but vascular complications are the risk every injector has to respect. If filler enters a blood vessel it can disrupt blood flow, which is why technique and anatomy matter more than the product name.

• Who injects? At WELLS it is a dermatologist, every time.
• Needle or cannula? We use a cannula where possible to reduce vessel-damage risk.
• Which product and how much? We confirm both before treatment, and the quantity is guaranteed.
• Is it hyaluronic acid? A hyaluronic acid filler can be dissolved if an adjustment is ever needed.
• Is the plan about your whole face? A forehead shaped in isolation can unbalance the brow and midface.
