Why is Korean PLLA filler different to plan from a regular filler trip?

Most people searching for Korean PLLA filler have already read that it builds collagen instead of just filling. What they haven't worked out is how that fits a trip to Seoul, because PLLA doesn't give you the finished face on the day you fly home.

I'm Won Woo Choi, a chief director at WELLS in Apgujeong. I completed my dermatology residency and board certification at Seoul National University Hospital and hold a Ph.D. in dermatology from Seoul National University.

What you will and won't see before your flight home

Right after a Sculptra session the treated area looks fuller, but that is mostly the fluid the powder is mixed in. As the fluid is absorbed over the next days, the volume drops again. That dip is expected, and I tell every patient about it before we start.

The real change comes from your own tissue. From about 4–6 weeks, new collagen starts to fill in, and it keeps developing over the following months. So on a one-week trip, you'll leave with the treatment done but not the result.

How many sessions does Korean PLLA filler take, and how do I fit them into trips?

About three repeat sessions are generally recommended for PLLA, because collagen builds in stages. The collagen formed usually lasts more than two years, although that depends on how quickly your own skin ages.

The listed price is for one session. At consultation the dermatologist assesses your face and tells you honestly whether one session is enough or whether a series makes sense.

What a realistic travel plan looks like

• Visiting Seoul several times a year: we space sessions across your trips and re-check volume at each one.
• One trip only: we can still do one well-planned session, and you should expect a gentler change.
• Combining with a lifting device: Sculptra and devices like Ultherapy or Thermage act on different layers, so the dermatologist plans the order and timing.

At every return visit we check how much volume has changed and how much collagen has formed, then adjust the amount and the areas. Skipping that check is how imbalance or unwanted volume creeps in.

Filler in temples and cheeks to lift: where Sculptra Lifting fits

Many people search for filler in cheeks to lift jowls, or filler in temples to lift eyes. The instinct is to fill the part that sags. Filling the sagging area directly can make the face look heavy, which is the problem our post on bad cheek fillers describes.

Sculptra Lifting is our WELLS-only technique for this. Instead of injecting into the sagging area, the dermatologist maps the face's support structure and places Sculptra at the temples and upper face, supporting tissue upward along the face's own lifting vectors.

It suits sagging cheeks and mid-face, a less defined jawline, and hollow temples or a flattened upper face. It's listed at ₩1,320,000 (VAT excluded) per session.

How we keep PLLA smooth: preparation, depth and 5-5-5 at home

The complication people worry about with Korean PLLA filler is a nodule, a spot where particles clump and feel hard. Most of the prevention happens before and during the injection, and the rest is in your hands afterwards.

• Hydration time: we don't mix the powder hastily just before treatment. We keep to the full hydration time so particles disperse evenly.
• Depth: too superficial raises nodule risk, too deep halves the effect, so we inject in layers with a micro-cannula.
• Area control: mobile areas around the mouth and eyes need much finer control of dose and depth. In thin under-eye skin, particle-based stimulators call for a very cautious approach.
• Dilution: more dilution means more cc but a weaker concentration of PLLA, so we plan by what's in the vial, not the volume in the syringe.

Then comes the 5-5-5 rule: gently massage the treated area for 5 minutes at a time, 5 times a day, for 5 days. This is easy to do in a hotel room or on the flight home, and we show you the exact spots before you leave.

Before planning, we use 3D volumetric analysis to find volume loss and left-right asymmetry that the eye misses. Dr. Lee, Dr. Ryu and I all completed Handok's Sculptra hands-on training program.

What about pillow face and forehead filler?

Pillow face usually comes from stacking volume where the face sags. PLLA works differently because it restores overall thickness gradually, and that's one reason I often prefer it when the whole face is slowly hollowing and looking tired.

Forehead contour is a different job. For shaping a specific area, such as the forehead, under-eye, nasolabial folds, cheeks or chin, we use dermal fillers with a cannula where possible, and we confirm the product and amount before treatment.

PLLA is not the answer for everyone. If you want a sharp, immediate change in one spot, filler is usually the better tool. If you want your face to look rested over the coming months, Sculptra is worth discussing.
