Ultherapy side effects: what's normal, what isn't, what lowers risk
A dermatologist's guide to Ultherapy side effects: what is expected afterwards, what is not, and how planning and staying awake lower the risk.

Ultherapy side effects: the short answer
Hello, I'm Dr. Won Sin Lee, a board-certified dermatologist and chief director at WELLS Clinic in Apgujeong, Seoul. I trained in dermatology at Asan Medical Center, where I also serve as an adjunct professor, and I co-founded WELLS in 2008. Patients often ask me about Ultherapy side effects before they book, so this is the answer I give in consultation.
Most of what people feel after Ultherapy is short-lived and expected: some redness, mild swelling and tenderness where the energy went in. The side effects that matter, burns and cheek hollowing, are mostly about how much energy is delivered and where, which means they are largely a planning problem.
That is why this guide is less about a list of symptoms and more about the decisions behind them. Individual results vary and side effects are possible with any treatment, so every plan at WELLS starts with a dermatologist consultation.
What is normal after Ultherapy treatment?
To understand what's normal, it helps to know what Ultherapy does. Focused ultrasound passes through the skin surface without damaging it and concentrates at points in the SMAS, the deep fascia layer a surgical facelift also tightens. Like a magnifying glass focusing sunlight, each point creates a small zone of heat.
The tissue contracts, and collagen regeneration continues during the natural healing that follows. Because that is real heat inside the skin, a few reactions are generally expected afterwards:
- Redness on the treated areas, which for many people settles quickly
- Mild swelling, especially along the jawline and under the chin
- Tenderness when you touch or press the treated areas
- A feeling of warmth or tightness for a short time after the session
These are the body's normal response to focused heat, not signs that something went wrong. If you're flying home soon afterwards, tell us your dates so the dermatologist can advise on timing.

Which Ultherapy side effects are not normal?
Two problems are worth understanding before you choose a clinic, because both are tied to technique rather than bad luck. Neither is a reason to fear the treatment; both are reasons to ask how it will be planned.
Burns from energy pushed too high
A burn is not an expected part of Ultherapy. The usual route to one is energy delivered higher than the tissue can take, and that becomes more likely when the patient cannot react. Under deep sedation you can't give feedback, so energy tends to be pushed higher and burn risk rises.
Cheek hollowing from energy in the wrong place
The side effect patients fear most is a hollow, gaunt look in the cheeks. It happens when excessive energy is delivered to areas that have little fat, and it can also leave the facial surface looking uneven.
Flat photographs and the naked eye miss subtle volume differences, which is why I don't plan Ultherapy from a mirror. More shots are not automatically better; the count should follow your anatomy and fat distribution.

Send a photo on WhatsAppWhy we do Ultherapy awake: sedation and safety
Many clinics put patients to sleep for Ultherapy. We don't, because Ultherapy's own guidelines don't recommend deep sedation, and because your reaction is part of how the dermatologist sets the energy safely.
Instead we use Air NOX, a controlled mix of oxygen and nitrous oxide, together with local anesthetic. It calms the nervous system while you stay conscious, and you can control the intensity yourself by breathing.
- No fasting and no recovery room; you recover within about two minutes and can drive afterwards
- You can see the tip and the shot count yourself
- Single-use sterile filters and mouthpieces
- Not suitable in early pregnancy, respiratory conditions, panic disorder or vitamin B12 deficiency, so tell us about these early
Working within your tolerable range lets us deliver adequate energy at the right points. Under sedation, energy is often lowered and more shots are added to make up for it, which is the opposite of what a careful lift needs.
How we lower the risk of Ultherapy side effects at WELLS
Safety with Ultherapy comes from a handful of decisions made before the first shot. These are the ones we make for every patient having Ultherapy Prime at our clinic.

- 3D imaging first: your face is analyzed with a 3D imaging system, and an individualized treatment map sets the energy and shot distribution for each area, steering away from fat-poor zones
- Ultherapy Prime system: the display shows the skin layers clearly, with ultrasound viewing down to 8 mm, so energy can be aimed where it is needed
- Genuine tips only: if a tip is ever found not to be genuine, we refund ten times the treatment price
- Dermatologist-led: a board-certified dermatologist handles your consultation, diagnosis and treatment
Shot count is part of safety too. Most of our patients are satisfied with 300–400 shots, and only about 2% of WELLS patients need 600 or more. WELLS is also a Merz Ultherapy Golden Record Award clinic.
You can read more about each of us on our dermatologists page, and the lifting and anti-aging overview shows every device on our menu.
Thermage vs Ultherapy: are the side effects different?
People comparing Thermage vs Ultherapy often ask which one is gentler. They work differently: Ultherapy places focused ultrasound in points at the deep SMAS layer, while Thermage delivers RF heat more broadly without wounding the skin surface.
That difference shapes the risks. With Ultherapy, the main questions are depth, energy and avoiding fat-poor areas so the cheeks keep their volume. Thermage has its own list, which I'd rather not compress into one line; our Thermage side effects guide covers it properly.
If you're still deciding what Ultherapy is and whether you need it at all, start with Ultherapy explained. It covers what it lifts, who it suits and how shot counts are decided.
Ultherapy Prime FIT, price, and what to ask any clinic
Because Ultherapy delivers energy point by point, small gaps can remain between them, and swelling can appear afterwards. In Ultherapy Prime FIT, a second device targets the layer between Ultherapy and Thermage depth, and its heat aids circulation, which can ease that post-Ultherapy swelling.
| Option | What it includes | Price |
|---|---|---|
| FIT 400 | 300 Ultherapy shots + Fitting 100 kJ | ₩1,925,000 (VAT excluded) |
| FIT 500 | 400 Ultherapy shots + Fitting 100 kJ | ₩2,409,000 (VAT excluded) |
| Ultherapy Prime alone | Shot count set at consultation | Confirmed at consultation |
FIT is not for everyone; it depends on face shape, skin thickness and degree of sagging. Whether you come to us or not, these questions will tell you how a clinic handles Ultherapy treatment safely:
- Who performs it: a dermatologist, or someone else?
- Will I be sedated, and if so, how is the energy checked?
- How is my face analyzed before the shot count is set?
- Can I see the tip and the shot count?
- What does the price include, and is VAT extra?
If you're planning the treatment around a trip, our Ultherapy Korea guide walks through the visit from WhatsApp to your flight, and the FAQ for international patients covers booking and prices.

What are the common Ultherapy side effects?
Redness, mild swelling and tenderness on the treated areas are generally expected after focused ultrasound. They are the skin's normal response to heat and, for many people, settle on their own.
Can Ultherapy cause cheek hollowing?
It can when excessive energy is delivered to areas with little fat. We analyze your face in 3D first and set the energy and shots per area to avoid fat-poor zones.
Will I be sedated for Ultherapy at WELLS?
No. We use Air NOX with local anesthetic, so you stay awake, need no fasting and can drive afterwards. Under deep sedation you can't give feedback, which tends to push energy higher.
How many Ultherapy shots will I need?
The dermatologist decides at consultation. Most of our patients are satisfied with 300–400 shots, and only about 2% of WELLS patients need 600 or more.
What should I do if something feels wrong after treatment?
Contact the clinic that treated you and send a photo. If you were treated at WELLS, message us on WhatsApp and the dermatologist will advise you.
Results and recovery vary by individual. Every treatment carries possible side effects, which are explained at the consultation. Prices are in Korean won, excluding 10% VAT, and confirmed at your consultation. This article is general information from the clinic, not a diagnosis.
