Does Thermage melt fat, or does it tighten skin?

Hello, I'm Dr. Won Woo Choi, a board-certified dermatologist and Chief Director at WELLS Clinic in Apgujeong, Seoul. I hold a Ph.D. in dermatology from Seoul National University and am an adjunct professor in its hospital's dermatology department.

One question comes up in almost every Thermage consultation with visitors: does Thermage melt fat, and will it make my face thinner or hollow? It is a fair worry, because heat-based treatments are often described loosely.

The short answer: Thermage is a skin-tightening treatment, not a fat-reduction treatment. Thermage FLX heats the dermis, from the epidermis down to the dermis, so collagen tightens and then rebuilds. When fat is the real concern, we use a different tool.

What a 2021 Saejeonbuk Shinmun feature said about Thermage FLX

In May 2021, Saejeonbuk Shinmun (새전북신문) ran a health feature on spring skin, which is part of the WELLS press archive. Its focus was pollen season: from late April to mid-May, pollen can trigger itching, dermatitis and flare-ups of atopic skin on the face, neck and arms.

The feature's advice was practical. Avoid the trigger where you can, shake pollen off clothes before going inside, and wash exposed skin as soon as you get home. Strong spring UV and fine dust can damage the skin barrier even in people without allergies.

For itchy skin it suggested a gentle, weakly acidic cleanser and a short shower rather than a long hot bath, which dries skin and makes itching worse. Clean indoor air and the right humidity help too.

Then it turned to treatment. It described Thermage FLX as delivering radiofrequency energy to the dermis to encourage collagen to contract and regenerate, improving firmness and lines around the eyes and mouth. Notice what is missing: nothing in that description is about dissolving fat.

Which layer does Thermage FLX actually heat?

Skin has layers: the epidermis on top, the dermis beneath it, then the fat layer, and deeper still the SMAS that a surgical facelift tightens. Each lifting device is chosen for the layer it reaches.

So, does Thermage melt fat? Its main target is the dermis, where collagen lives. Heat tightens the existing collagen, and new collagen builds over the following weeks. That is why the change is firmness and texture, not volume loss.

Will Thermage make my face look hollow?

This is the real fear behind most 'does Thermage melt fat' questions. People who already have a slim face worry that any heat treatment will take away the soft volume in their cheeks.

Because Thermage works on the dermis, it is not designed to reduce the fat layer. What matters more is matching the treatment to the face. For a very thin face with deep folds, I may recommend a different plan, such as Sculptra to support the contour from above.

Why tightening is often mistaken for fat loss

Part of the confusion is visual. When loose skin along the jawline firms up, the lower face can look slimmer in photos, and people describe that as Thermage fat loss. What has changed is the support of the skin, not the amount of fat beneath it.

The other source is marketing that groups every heat device under one word. Radiofrequency, focused ultrasound and microwave energy reach different depths, and that difference is the whole point of choosing one over another.

So when someone asks me 'does Thermage melt fat', I ask a question back: what exactly do you want to change? If it is crepey texture, fine lines or early laxity, skin tightening with Thermage makes sense. If it is a full double chin, heat on the dermis alone will not address it.

• Loose, thin skin with fine lines: Thermage FLX is a good fit.
• Sagging along the jawline from deeper layers: Ultherapy Prime FIT often fits better.
• A soft jawline or double chin with fat under it: Onda MAX reaches the fat layer.
• Loose skin and some fat together: a combination, planned in person.

If fat is the problem, what does WELLS use instead?
